A crew was utilizing a forklift to install fence barriers around a well. The carriage of the forklift was being used to push down on the fence frame and push the legs of the barriers into the feet slots in the ground. The injured employee was signaling the forklift operator with his left hand while his right hand was on the barrier next to the one being installed. The forklift was pushing down on the barrier when the carriage slipped and contacted the injured employee's right index and middle fingers, resulting in amputations.

An employee arrived on location and loaded a blow-out-preventer (BOP) on a 1-ton flatbed truck. While standing on the flatbed securing the the BOP, the employee stepped into a void/hole on the flatbed surface. This caused him to lose balance and fall off the truck bed with his leg still caught in the hole. As a result, he sustained fractures to his left leg.

An employee was pulling and positioning the first joint of pipe (30-foot joint of 2-7/8 tubing) at the end of the pipe racks so that it could be lifted at a later time. The pipe was stacked three layers high. The pipe from top layer rolled and three joints of pipe fell at the opposite end of the pipe racks. The employee reached to stop the pipe from falling at the end where he was standing and his right little finger was crushed between the pipe and the pipe rack pin. The employee's finger was amputated.

During maintenance of an annular blowout preventer, air pressure was applied to dislodge a stuck internal piston. The piston ejected and struck an employee's left bicep, causing a laceration and a fracture.

An employee was working on a rig crew. They were positioned on the rig work floor, helping to connect tubing to a power swivel. While the employee was making a horizontal connection, their left hand was positioned on the winch line that was secured to the power swivel. As the swivel rotated, the winch line chain became tangled, pinching the employee's left little finger and resulting in an amputation.

An employee was working to build an electrical substation. He was cleaning out the inside of a fixed section of a PVC pipe on the end that was furthest from an excavator. The excavator was outside of the trench and more than 25 feet away when it moved a pipe, pinching the employee's right arm between the two pipes. The employee's forearm was fractured.

An employee was anchored to a movable piece of equipment called a "lid" with a self-retracting lifeline harness. An overhead crane lifted the equipment, causing the employee to be pulled by his harness. The employee was pulled off the walkway and swung over the wall of the kiln. His body and head struck the wall. The employee lost consciousness and sustained a laceration to the side of his head that required stitches. The employee also sustained multiple fractured ribs and fractured vertebrae.

An employee was in a scissor lift, preparing to strip formwork from the underside of a concrete deck. As the lift was ascending, the employee's left little finger was pinched between the lift rail and the concrete beam above. The employee was hospitalized, and the soft tissue of their left little fingertip was amputated; the finger's bony tuft was also damaged.

An employee was operating an order picker when his left middle finger was crushed between the lift and storage racking. The employee's fingertip was amputated.

An employee was working to install a drive belt on a water pump. He was holding the belt on both sides of the pulley, working to route it over the top. As he pulled down with his right hand, the pulley rotated and the belt slid over it, pulling his left little finger between the pulley and the belt. He suffered an amputation to the fingertip as well as a fracture to the ring finger.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
