An employee was discarding fryer oil into a bin by the dumpsters when oil spilled onto him, burning his right ear, face, chest, back, left arm, and left leg.

An employee was cleaning a double burner when it fell and crushed their right middle finger, resulting in a partial amputation.

An employee was on the ledge of a piece of fryer equipment, changing the vent filters above the hood. Her right foot slipped into the hot oil, and she suffered third-degree burns to the calf, ankle, and foot.

An employee was standing on a step stool and attempting to grab an empty pot from on top of an oven when the step stool collapsed and slipped out from under her feet. As she was falling to the ground, her left hand grabbed a pot of boiling water from a stove below, spilling the contents on her body. She suffered third degree burns to her stomach, chest, left arm, elbow, and left hand.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee pulled out a bucket of hot oil from under the fryer. The employee then stood on the table to clean the back wall. He stepped down into the bucket of hot oil, resulting in third-degree burns to his left leg.

An employee was walking through the retail store when they tripped over a display rack and fell. The employee was hospitalized with fractures to their left shoulder, left elbow, and left wrist.

An employee was walking toward the dish area when they tripped over a fan cord and fell on the ground. The employee sustained a hip injury.

An employee was shocked while turning on a circuit breaker for a dishwasher that needed repair. The employee was hospitalized.

An employee was on a roof servicing a refrigeration system. They were using a ladder to get off the roof when they fell, resulting in a shattered left heel that required hospitalization and surgery.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
