On February 27, 2020, an employee fell to the ground outside near welding gas cylinders, suffering a head injury. The employee was found unconscious and was hospitalized.

On November 16, 2016, an employee was retrieving parts from a new washer when his glove was caught in the chain conveyor and subsequently caught between the chain and a rotating shaft. His right middle fingertip was amputated. The chain and shaft were unguarded at the time of the incident.

DHL Express USA Inc.

An employee was operating a forklift on the airport ramp environment to help offload cargo pallets from a tractor trailer. He was manually locking the cargo dolly (a specialized piece of ground support equipment used to transfer pallets) to prevent the load device from shifting or falling off the dolly during transportation. The employee stepped between the K-loader and the dolly to put up a lock when they became pinned between the loader and the dolly, resulting in inflammation to their left leg. The employee was hospitalized.

An employee had been loading a trailer. Their foot was caught in the trailer and/or trailer hitch, resulting in a severe laceration to a toe that required surgical amputation.

The injured employee was standing on a form/bed where concrete is poured and was removing concrete from the form(s). Another employee indicated through hand signals to the injured employee that the chocks had not been removed from the strands. The employee jumped down from the bed to ground level. The bed then started moving toward him. He went to jump over the drive wheel to get out of the way of the bed, but his foot got caught in the drive wheel. The employee sustained a severe laceration and crush injuries to the foot that required surgery.

An employee was making repairs to a milling machine located in the mold shop. While using a screwdriver to make adjustments, the machine energized and struck the employee. The employee's right arm was fractured.

An employee was breaking down dunnage to prepare totes and trays to go through a tote washer. A tow motor driver asked the employee to move a cart that was in the way so the tow motor could remove a gaylord. The employee moved the cart and then stepped away from the tow motor. As the employee moved to an area between carts, his right foot got caught by a cart, causing him to twist and fall on top of a cart, landing on the left flank of his torso. The employee suffered a hemorrhage on his left flank.

Three employees were installing a transfer rail in a press. There were two employees holding the rail and the injured employee was at the end of the rail guiding the rail into the bracket. The task was done manually with no mechanical equipment. The employee had their finger at the end of the rail and went to make sure the rail was lined up while the other two employees pushed it into the bracket, causing their finger to become pinched between the rail and bracket. The employee's middle fingertip was amputated. Protective gloves were worn at the time of the incident.

An employee was walking across the dock, interacting with forklift drivers on what was being loaded. A forklift drove forward with a tall load on the forks. The employee was struck by the load (running board/rocker panel rack) on the forklift. The employee sustained several contusions and an injury to their right Achilles tendon that required surgery.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
