Termax Corporation

An employee was loosening a bolt from a cylinder in the plastic injection molding machine. When he attempted to grab a hand jack, the plate came down because the bolt was loosened, and the employee's right hand was pinched between two metal plates. When he pulled his hand out, his pinky finger got lacerated and required surgery.
