He was removing damaged product when his right hand was caught between the head sealer and the bottom seal plate on the form-fill-seal packaging machine. The employee sustained amputations to his little and ring fingers. The machine was not locked out/tagged out or guarded at the time of the incident.

An employee was operating a small tabletop grinder in a food safety quality assurance lab. She was stuffing meat down the grinder's chute when the grinder caught her right hand. Its auger caused an amputation to her middle finger and a partial amputation to her index finger.
An employee was cleaning meat from within the x-ray portion of a purge machine. The gate of the machine activated, and its valve amputated the employee's fingertip below the fingernail.
