An employee was setting chains to secure a concrete barrier to a front-end loader. The chain became twisted and lost slack. The concrete barricade fell on the employee's left foot, causing a partial amputation to a toe.

An employee was standing on a ladder installing a fluorescent light bulb ballast when he contacted a live wire and received an electrical shock to his left hand, causing him to fall from the ladder to the floor. The employee was hospitalized with an electrical shock and burns to his left hand, as well as fractures to his right tibia and distal right radius. The wire was not deenergized or locket out/tagged out.

On January 25, 2022, an employee was preparing an injection needle and was stuck on his finger. The finger became infected and the employee was hospitalized.

RG Professional Carpenter, Inc.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

Rae Corporation

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

Red Dot Corporation

An employee was attempting to lift a steel beam from a horizontal, stacked position to a vertical position using a crane. They had placed two hooks on the east side of the beam on the flange. As the employee was hoisting the crane, the beam began to shift and pulled the employee forward onto the stack. When the material shifted, the hooks released and the material fell, pinning the employee between the two beams. The employee was hospitalized with soft tissue contusions on their proximal right thigh and interior left thigh.

An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.

She then took a step back and her right shoe got stuck/stayed gripped on the floor, causing her to fall on her right side. The employee was hospitalized with a fractured right hip and required surgery for her right wrist.

On September 25, 2025, an employee climbed an aluminum extension ladder and was on top of the roof of a building She was walking around on the roof, inspecting and evaluating it. She was descending the ladder when the ladder slid to the right and twisted around as she hung onto it. She then fell 10-12 feet to the paved parking lot and the ladder landed on top of her. She sustained a fractured left collarbone, multiple other fractures, and had bruising to the left side of her abdomen.

At about 5:55 p.m. on August 9, 2025, an employee lost his footing while climbing an access ladder to a 12-foot-high spotlight platform. He fell about 9 feet to the ground, breaking eight ribs on the right side.

On August 1, 2025, an employee was preparing to initiate canine training when they were bitten by a dog on the neck and face.

An employee was completing an annual work capacity test. Later that night, the employee experienced severe pain in their left leg and was hospitalized with rhabdomyolysis and compartment syndrome in the lower left leg.
