A driver slipped and fell to the ground while exiting his truck. He sustained a lacerated spleen.

An employee was operating a ride-on sweeper in the yard cleaning the concrete pad. He took a turn toward the radial stack conveyor and the sweeper struck a C-channel support beam on the side of the conveyor. The employee sustained lacerations to the face/neck, a compression fracture of the T-7 vertebra, and a punctured lung.

An employee was attempting to clean off a machine's sensor when the machine started to move. As he attempted to remove his hand, his right middle finger was caught in a chain and sprocket, tearing off his fingertip. The machine was not locked/tagged out at the time.

An employee was observing a yard horse after replenishing hydraulic fluid to the lift mechanism when overpressure in the fluid reservoir caused a hot fluid spill. The fluid contacted the employee, causing thermal skin burns to the face and abdomen.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

After clearing a plugged cement feed chute underneath a silo, the injured employee asked a coworker to "bump" the rotary feeder to confirm material flow had resumed. The rotary feeder's inspection port was open, and dust was escaping until the injured employee covered the hole. Material then built up and prevented a visual inspection inside the feeder. The injured employee then instructed the coworker at the operating switch to stop the machine. While the feeder was still rotating due to inertia, the injured employee pushed the packed material out of the inspection port. A rotating blade partially amputated their right index fingertip.

Liquid concrete was being unloaded from a ready-mix truck into a mobile dump buggy. An employee was holding the truck s chute handle while standing beside the buggy. As the dump buggy moved forward, the employee s fingertip was caught and pinched between the chute handle and the buggy s bucket. The left ring fingertip was amputated.

An employee was utilizing a 10-foot fiberglass ladder to install an antenna on the side of a one-story residential home. The height of the mounted antenna from the ground was approximately 12 feet. While completing the installation of the antenna the employee lost his balance and fell approximately 8 feet to the concrete below, resulting in two fractured heels.

An employee was on a flatbed straightening return pallets when they sustained a back injury.

An employee was unclogging a cement silo when the butterfly gate closed on their left thumb and crushed it, resulting in a laceration.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
