Two employees were troubleshooting a post-fire event in a pump room. It was found that one or two nuts were missing from the bolts that secured the loose blow-out panel. One of the two employees placed the blow-out panel back in its original position. As he closed the oil chamber, the vacuum increased. Mixing conditions changed and the oil dispersion exploded. The two employees in front of the blow-out panels sustained severe burns.

An employee was installing personal protective grounds at a substation when their hand contacted the end of the ground that was not energized, but induced voltage was created by wind and shocked them. This resulted in a burn on their thumb.

An employee was installing a power pole when his finger got caught in the chain that was lifting the pole, resulting in a left ring fingertip amputation.

An employee was standing on a truck when an overhead powerline broke and landed on the truck. The employee sustained an electrical shock.

On September 15, 2021, an employee was pulling wire out from under some bushes when he tripped and fell, resulting in a rib fracture and a punctured lung. He was hospitalized.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was operating a stand-up forklift. The forklift drove into a rack in a warehouse, and the employee's right hand was caught between the rack and an upright supporting the forklift's overhead guard. The employee suffered a fracture and laceration to the hand, just below the little finger.

An employee was holding a well head cap preparing to pack a wall off when the slips holding a pipe failed and the pipe came down on his fingers, resulting in an amputation.

An employee had been driving in a pasture to check on a pregnant cow. The truck door closed on the employee's hand causing an amputation to the right middle fingertip that was closed with stitches.

An employee was stung by a bee and had an allergic reaction.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.
