An employee lost his footing while climbing an inclined ladder. He stumbled down one step and was able to stabilize himself by holding the railing and twisting his body, but he suffered an internal abdominal injury and was hospitalized.

An employee was climbing a ladder to inspect the roof of a church when the ladder slipped and the employee fell. He went to grab the rain gutter along the eave to break his fall when his right hand contacted a sharp edge on the gutter, resulting in a laceration to the right-hand fingers that required hospitalization.

An employee was performing preventive maintenance activities that included cleaning coils on a turbine oil cooler. The employee used a 6-foot step ladder to access the mid-flange area, where he stood to access the oil cooler, apply silicon in-between the flanges, and bolt up the flanges. As he was standing on the side of the cooler, he fell from a height of less than 4 feet but managed to catch himself during the fall. In doing so, his left triceps contacted the threads of an existing stud that secured the top cover, resulting in a puncture wound that required hospitalization.

An employee was preparing a commercial jet-liner to be repainted. He had accessed the wing via a scissor lift, and was standing on a flap over an aft wing box, at a height of approximately 15 feet. His harness was on, and he was hooked up to the scissor lift yoyo. He slipped off the top of the wing and fell approximately 6 feet, making contact with the plane and scissor lift, resulting in a fractured left femur.

An employee was descending a fixed ladder into a block house to perform maintenance on a snow-making system. His foot slipped and his heel struck a pipe (part of the snow-making system) behind him. The employee fell partially down the ladder before he stopped himself from falling further and another employee helped him out of the space. The employee suffered a broken right tibia and fibula.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was on a ladder replacing a net on a basketball hoop. When he took a step up, he lost his balance and the ladder started shaking. The employee went to hang onto the back of the rim but fell. As the employee fell, his leg got caught on the ladder, resulting in a broken leg that required hospitalization and surgery.

A maintenance employee was on an A-frame ladder replacing a ballast light in the locker room. The employee suffered a shock and fell approximately 5-6 feet off the ladder to the floor below. The employee sustained a fractured hip that required hospitalization and surgery.

An employee was moving equipment as a part of closing duties when the cooler amputated their left index fingertip.

An employee opened up the bottom grate of the washing machine to adjust the microswitch drive on the unbalanced microswitch. Once they adjusted the rod on the microswitch, the washing machine started back up. The employee sustained a complete amputation of the left arm to the middle of the humerus and lacerations to the head.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was shot in the leg with a pistol during an altercation with two people who had entered the store without permission.

An employee was directing a truck driver to a dump site when the truck's peanut trailer slipped off the kingpin. As the trailer fell, a ladder attached to it lacerated the employee's ear. The employee was hospitalized.

An employee was unloading a forklift from a trailer to the ground. The employee was climbing down the ladder of the trailer when their foot slipped through one of the rungs, causing them to fall backward onto the forklift forks. The employee was hospitalized for fractured ribs.
