An employee was emptying a dumpster into a rear-load trash truck at a customer location. The employee's left index finger got caught by the dumpster as it was being dumped, resulting in amputation of the finger at the knuckle.
An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.
