An employee was painting a round module when a front-end loader ran over his right leg and crushed it.

An employee was operating a switch truck at a rail yard. He got out of the switch truck to unlock a container that was attached to a unit in front of him. The switch truck rolled and pinned the employee against the container. The employee sustained a fractured pelvis, a ruptured spleen, and a possible spinal cord injury.

On October 16, 2025, an employee was entering a parking lot after completing their work shift when they were struck by a vehicle being operated by a customer. The employee fell backward from the impact and the back of their head struck the asphalt. The employee was hospitalized due to brain bleed and a skull fracture.

An employee was moving branches and other material when they were struck in the leg by the track of a skid-steer loader, resulting in an ankle and lower leg injury.

An employee was waiting for an operator to bring mail over to a mail sorting machine when she became pinned between the machine and a stack of pallets being pushed by a powered industrial truck (PIT). An employee was coming from the patio area to replace a trash can. He was carrying the trash can and walking toward the entrance of the cafeteria when he tripped over the rubber mat that precedes the ramp. He fell to the ground and sustained a torn ligament in his left shoulder.

An employee stepped out of her office, turned to lock the door, turned back, slipped, and fell. She suffered a broken right femur and was hospitalized. There were water droplets on the floor from a wet umbrella.

When he reached up to pull a wire, the ladder slid down the wall to the floor. The employee fell on top of the ladder and struck the floor, fracturing multiple bones in his face and his left hip.

An employee reached through the vertical bars of a powered gate to enter the code on the keypad. The gate opened and the employee's hand was caught in the bars. The gate moved for 5 to 7 seconds and the employee was dragged 3 to 5 feet. The employee sustained a compound fracture of the lower right arm.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.
