Final narrative
Two employees were moving a 6-by-6-foot concrete pull box and using a machine to reposition it. To protect the concrete from the machine, a 4-by-6-inch piece of timber was placed between the machine and the box. The two employees were on either side of the box holding the timber in place as they moved the machine into place. As the machine approached the timber, the timber rolled and grabbed the injured employee's glove, resulting in amputation of their left index fingertip as well as a bruised fingernail on another finger.

A crew was installing drilled micropiles alongside an outdoor covered deck foundation. The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing.
