An employee was clearing a jam in a fish skinning machine when his hand became caught between two plastic cogs that operate the conveyor belt. His hand was injured and required surgery. His left index finger was surgically amputated. The machine was energized at the time of the incident.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.
