An employee was clearing a jam in a machine. The employee's right thumb became stuck in the machine and the employee suffered a laceration and removal of the nail.

An employee had been sanitizing a portable conveyor belt. While moving a section of the conveyor system, the employee's right index finger was caught by a tire of the conveyor system and their fingertip was amputated.

A temporary employee was tasked with releasing water from the pasteurizer. As she opened the main double doors, boiling water was released and the employee suffered burns to her hands and legs while closing the doors.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

An employee was disassembling a pressurized unit using a driver drill. When the unit was disassembled, it caused a kickback from the tube head, causing the drill to strike the employee's left forearm. They sustained a diaphyseal fracture to their left radius and ulna, which required surgery.

An employee was using a box cutter when it cut the tip of his left thumb resulting in a fingertip amputation.

While working at a customer's residence, an employee was cutting tape with a box cutter to wrap insulation for a piece of pipe. The box cutter struck a hardened object within the wall causing it to bounce back and puncture the employee's right eye. The employee was hospitalized.

An employee was using the pipe-and-wrench method to loosen a seized bolt on a roof truss. A technician from another company was on the other side of the truss doing the same action with the same method. The employee's socket slipped off the bolt and his left hand was crushed against the truss, resulting in injuries to the index and little fingers, requiring hospitalization, surgery, and amputation of the index finger.

An employee was working on a fillet machine when it caught his right little finger, lacerating and breaking it.

An employee was cutting a fish hung up on a hook using a knife when the hook came off, causing his hand to slip. The employee's right thumb was severely lacerated by the knife.

An employee was clearing a jam in a fish skinning machine when his hand became caught between two plastic cogs that operate the conveyor belt. His hand was injured and required surgery. His left index finger was surgically amputated. The machine was energized at the time of the incident.

An employee was in a tunnel freezer, which was being turned on for the first time. The baffling for the evaporator fan got stuck in the siding. The employee ascended a 5'9"-tall cart to reach the raised baffling panel. The raised baffling shifted and dropped, trapping the employee's arm between the aluminum and foam core baffling. The employee sustained a severe laceration to the lower forearm/upper wrist and severed ligaments. The employee was hospitalized.

On August 7, 2024, a contracted employee was operating a fish skinner that uses two rotating stones to skin catfish when fingers on her right hand were caught by the stones and severely injured, resulting in hospitalization. The employee sustained an open wound of a finger with damage to the nail/nailbed and an open fracture of the distal phalanx.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
