An employee was using a rag to clean oil off a tube expander machine. A roll pin that holds the chuck on the roll machine caught the rag, and the employee's hand was pulled into the roll gun, resulting in a finger amputation.

CUST-O-FAB Specialty Services, LLC

An employee and a co-worker were loosening nuts and bolts on a 24-inch, 300-pound flange with hydraulic torque equipment and a reaction arm. The hydraulic torque equipment was not functioning due to pump issues. The co-worker went to inspect it when the pump kicked on and engaged the torquing equipment. The torquing equipment moved the reaction arm, which struck the employee's right-hand index finger, resulting in a fingertip amputation.

An employee was on the working platform of a scaffold at a height of approximately 9 feet doing welding operations. During an alarm, the employee fell from the scaffold's ladder to the ground, resulting in a compound fracture to the lower left leg requiring hospitalization and surgery.

Employees were moving a heat exchanger tube bundle with a hydraulic crane. The crane had set the tube bundle down on a crawler and the employee had disconnected the synthetic sling from the bundle and used hand signals to the crane operator to rise the main line. The sling caught the tube bundle and rolled off onto the employee.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.
