An employee was prepping a work area on a scaffold, which was approximately 12 feet high. As he was adjusting a piece of rebar, he slipped and fell over the concrete block wall in front of him and onto the concrete floor below. He was hospitalized with contusions, abrasions, lacerations, and two broken arms.
