On June 27, 2025, at approximately 7:00 PM, an employee was operating a cell that presses rings on a cylinder and welds the rings on. The cylinder was moving between the welding station and the press station when it fell off the holder. The employee went to place the cylinder back on the holder when the cell cycled and the compression holder crushed their right hand. The employee's middle, ring, and little fingers were crushed.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

CFL USA, LLC

A temporary employee turned off a planer machine, opened the housing, and began clearing a jam. The machine's spinning blade caught his right hand and degloved the index finger.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

HYDAC TECHNLOGY CORPORATION, HYDRAULIC DIVISION

An employee was working in the shipping/receiving area on a packing paper dispenser that was jammed. When the paper came free, the blade pulled the paper in and pulled the employee's hand in too, amputating about a quarter of the right middle fingernail and the top of the finger.

Hannon Hydraulics, LLC

An employee was working on a hydraulic cylinder at his work table. While he was removing a bleeder plug to reduce the pressure, the 3/8-inch steel plug was pushed out by oil pressure. The plug impacted his wrist, causing an abrasion where hydraulic oil went into his arm. The employee was hospitalized.

An employee was pulling a steel cart backward when his left ring finger was crushed between the cart and another steel cart, resulting in amputation of the fingertip.

An employee was using a hydraulic press to repair a hydraulic tool that was stuck in the open position. Upon adding downward pressure from the hydraulic press, the tool's hydraulic quick coupler broke and released the ball bearing inside the coupler. The ball bearing struck the employee in the abdomen, resulting in lacerations and abrasions. The ball bearing also penetrated the employee's abdomen.

An employee was operating a manual tube lathe when a piece of metal stringer (twine rope) caught his left forearm, resulting in a laceration. The employee was hospitalized and had surgery.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

Foundation Steel, LLC

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
