Federal OSHA recorded a severe workplace injury at Great Hills Constructors, HWY 183, AUSTIN, TEXAS 78759 on — Injuries to the brain, spinal cord and severe wounds, internal injuries, affecting the Head and extremities.

An employee was picking up a traffic closure when an 18-wheeler rear-ended the truck-mounted attenuator (TMA). The impact spun the TMA sideways and forward; it struck the employee before impacting the barrier wall. The employee sustained a concussion to the right side of the forehead and a fractured left femur.

An employee was driving a form stake into the ground using a hammer while holding it in position with his right hand and wielding the hammer in his left hand. His right index finger was struck by the hammer, resulting in a compound fracture.

Gras Lawn LLC

An employee was in a coned-off work area on a street, performing a granular turf application. While he was standing between his spreader machine and a truck, loading the spreader with bags of fertilizer, a car drove by and struck the spreader. The employee was pinned between the spreader and the truck and suffered bruises, scratches, and cuts to his legs (particularly the left knee), arms (particularly the right elbow), and left shoulder. He was hospitalized.

Greggo & Ferrara, Inc.

A flagger was directing traffic flow in a highway work zone. They were standing approximately 1.5 feet behind the shoulder's demarcating line, next to and slightly behind a traffic drum. As they were directing traffic into and out of the driveway of a parking lot, a car attempted to get around a truck and struck the traffic drum, which subsequently struck the employee, who was thrown 15 feet and landed on top of concrete aggregate. The employee suffered pelvic and rib fractures, T10 and L3 vertebra fractures, and internal bleeding.

Entergy Texas, LLC

An employee was installing material on a utility pole before setting the pole into the ground. A vehicle entered the work area and struck the pole. The utility pole struck the employee in the leg and they were hospitalized with a fractured leg.

LDR Construction LLC

A construction crew was working on a bore section that crossed under an interstate. One of them was crossing the interstate, and was in the center median next to a cable barrier, when a tire came off a trailer being hauled by a truck and struck his left side. He suffered fractures to his leg and left hip, as well as lacerations to his body and lower abdomen. He was hospitalized, requiring surgery.

Archer Western - Snell JV

An employee was working on the protected side of a jersey barrier when a metal pipe (10 feet x 6 inches) fell off a passing commercial truck. The pipe was struck by another passing vehicle and was launched over the barrier. The employee was struck by the pipe, resulting in a fractured pelvis.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

Anderson Columbia Co., Inc.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

Warner Brothers, LLC

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

Don Martin Corporation

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

T G Meat Center LLC

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
