An employee was completing a winding process involving installation of magnet wire into a stator. The final loop of the wire wrapped around the employee's left little finger and pulled it into the stator, resulting in an amputation to the finger below the first joint (in the area of the fingernail).

An employee was monitoring a shaker during an outdoor shift when they suffered dehydration due to heat exposure, resulting in kidney failure and hospitalization.

An employee climbed on a frac stack of valves when an unbalanced load began to shift. As the employee went to stabilize the connection, they lost balance and fell 7 feet to a lower level. The employee sustained a left wrist fracture that required surgery.

He sustained a fractured tibia and fibula just below the left knee.

This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. The employee leaned over the machine and the lead screw caught his shirt, pulling him into the machine. As it pulled tighter, he was forced toward the machine, impacting his right shoulder. The employee suffered multiple fractures and torn tendons to right shoulder.

An employee was installing a blind flange onto a pump fluid end. The employee was pushing the flange into place when their left index finger was caught between the fluid end and the blind flange of the counterbore of the steel pump. The fingertip was amputated.

SIMFLO, LLC

At about 3:00 p.m. on October 28, 2021, an employee was on a work platform, which had been raised by a forklift 8-10 feet off the floor so he could access the control box for an overhead door that had stopped working. A door on the platform swung open and the employee fell backward to the floor, landing on the back left side of his body. He suffered six broken ribs on the left side and was hospitalized.

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.
