An employee was diagnosing the cause of a rear light malfunction on a forklift when the reverse gear engaged. The forklift moved backward and contacted the employee causing a metatarsal fracture in their left foot and injury to their lower right leg.

An employee was collecting shopping carts in the parking lot when he was struck by a customer's truck as it was backing out of a parking spot. The truck pinned the employee's left leg against the cart mule they were using. The employee was hospitalized with a fractured leg and required surgery.

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

An employee was standing on a step ladder picking apples from a dwarf apple tree while holding a bag weighing approximately 35 pounds. The employee slipped and fell approximately 4.5 feet to the ground, resulting in a fracture to the lower left leg. The employee was hospitalized.

An employee was working from a scissor lift to paint piping. The employee finished painting, lowered the lift, unlatched the chain on the back, and was standing on the top step of the ladder that is used to access the work platform. The lift raised approximately 6 feet into the air and the employee fell onto the concrete floor below. The employee sustained a concussion and a neck fracture requiring hospitalization.

An employee was tightening the gas cap on a tree trimmer that had been recently turned off but had gas spillage around the cap. The trimmer ignited, burning his hands, face, and chest.

An employee was performing maintenance on apple sorting equipment when his fingertip was amputated by the chain and sprocket as the machine started up.
