An employee was moving 8 rail tank cars onto the main lead railroad line and was preparing to connect car 8 to the train. The employee entered the space between the train and rail car 8, facing the train, and went to manually open the knuckle on the coupling. His right hand was on the knuckle and his left hand was on the uncoupling lever. The employee's right hand was pinched between the coupling knuckles of rail cars 7 and 8, amputating the right hand at or near the wrist.

An employee noticed that a four-high stack of new wheels were not stacked straight on a conveyor line. The employee grabbed the second wheel from the bottom and began to rotate the wheels to straighten the stack. As he began turning them, the conveyor began feeding forward and the wheels fell, pinching his left hand between the wheels. His index, middle, and ring fingers were broken.

An employee was attempting to install a roller set onto a track system for a rolling-style door on a box railcar. The door was raised and was being supported by a forklift. A sledgehammer was used to hammer the roller set onto the track assembly. The impact of the sledge caused the roller set to slide off the fork, dropping the door 3-4 inches back to its original height. The employee's left index finger was pinched between the fork and a vertical door support pole. The finger was amputated.

An employee was moving metal out of a cart. He had removed one piece and was working to move another when the metal shifted. His right ring finger was crushed between the two pieces, and he suffered a partial amputation to the finger pad at the tip.

A load of steel angles was being lowered when the lifting chain hit the load, causing it to slide toward an employee. An angle slid and pinched the employee, catching both of their legs between the beams and an angle. The employee suffered two fractured legs.

Employees were replacing a 24" pig trap door. A strap was shackled on the pig trap door and connected to a forklift. Welders began to cut the door off. An employee was holding onto the door when the cut was finished. The door shifted and caught his fingers, resulting in a hand injury and amputation.

An employee used a crane to set down a bar. The bar measured 9 inches by 16 feet. The strap got stuck under the bar, and the employee asked another employee to jog the roll. The bar rolled and pinched the injured employee's left hand, resulting in a fingertip amputation.

An employee was releasing air from an inflatable mandrel which was loaded onto a fork truck. The mandrel was moved by the fork truck and the employee's finger got caught in the hole where the air release button is located. The employee's fingertip was amputated without loss of bone.

An employee was sampling a railcar in a storage yard. After sampling the railcar, the employee was climbing down the railcar ladder when he fell off the top step of the ladder to the ground, resulting in fractures to six ribs, his right arm, and two vertebrae.

Employees were watering a train at a train platform. One of them went to step over a hose, tripped, and fell to the ground. He suffered a broken knee. The employee was hospitalized for surgery.

Employees were performing maintenance operations on a railroad line when the machine experienced a load conveyor motor failure. While clearing the machine to allow for repairs, an employee pinned the left side of the swing conveyor. The operator on the right side then requested a slight bump to the left to align the right side for pinning. Once the swing was in position, the operator went to pin the right side and the chain tensioner failed, resulting in an abrupt release of the swing chain. The chain snapped back and struck the operator s right hand, resulting in a 1-inch laceration and a fractured fifth metacarpal. The employee was hospitalized and required surgery on the hand.

The injured employee loaded a container onto a railcar and then mounted the railcar to put in the locking mechanism for the container. The employee was struck by another container that a crane was loading onto the railcar and fell approximately four feet from the railcar to the concrete ground. The employee sustained fractures to their pelvis, shoulder, and two ribs.

An employee was removing a power assembly from a locomotive engine using an overhead crane. The power assembly became stuck, and two 1-inch lifting bolts failed as the crane continued to apply lifting pressure. This cause the power assembly to come back down and crush the employee's right hand against the engine housing. He suffered two broken bones in the hand, as well as tissue and ligament damage, and was hospitalized.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.
