An employee was operating the cash register while customers were attempting to shoplift items. The employee was assaulted by five customers, resulting in fractured ribs requiring hospitalization.

An employee was monitoring a psychiatric patient who was walking in the hallway. The patient grabbed the employee by the neck and threw her to the ground. She was hospitalized with an acute superior endplate compression fracture with minimal retropulsion to her T12 vertebra.

A mail carrier was delivering mail when aggressive dogs approached. The dog's owner tried to restrain the dogs but one dog broke loose and the mail carrier pepper sprayed that dog. The dog's owner then pushed the mail carrier and he fell on the pavement and injured his neck. The mail carrier was hospitalized.

An employee was working on an assembly line in the production facility. He was on the south part of the aisle, checking for debris in the system, and went to remove a screw. His left foot became caught in a wheel on a track along the assembly line. All of the toes on his left foot were amputated.

An employee tripped while assembling machinery components and fell to the floor, suffering a broken leg.

A maintenance employee was using an air wand to clean equipment. The air wand became caught in the rollers that were turning and the employee's left arm was pulled into the equipment. Their arm was crushed to the elbow and the fifth digit on their left hand was amputated.

On July 3, 2024, at approximately 8:00 AM, an employee was installing a pulling sheave in an electrical gear when they received a high-voltage electrical shock and sustained arc flash burns.

An employee was cutting a brass punch on the band saw. When the employee went to clamp the brass part, the part moved causing the clamp to crush the end of their right middle finger and it amputated the finger between the first knuckle and the fingernail.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
