An employee was removing the bottom die from an auto hammer. He was pushing the die out with his left hand when the die bar slipped and fell on top of his left index finger, resulting in a fingertip amputation.

The forklift pulled back to have the electrical box slide off the forks. On the other end, employees were bracing the electrical box from tipping over. The box then fell forward, the injured employee slipped while evading the box, the box hit his lower leg and torso, and he fell fell backward. The employee was hospitalized with a fractured T12 vertebra.
