An employee was accessing the bed of a delivery truck using a ladder mounted to the side of the truck. He lost his footing, fell, and struck his head on the bed of the truck. The employee sustained a fractured skull. It was raining at the time of the incident.
A pallet of shingles was being loaded onto a flatbed truck. It broke and gave way, and the bundle of shingles slid off the opposite side of the truck bed and landed on the driver's lower leg. The leg was broken.
