An employee was inspecting a direct-current cable that was going to be pulled with the help of a vehicle. The cable was made from two cables coupled together that each measured 2 inches in diameter. The cable came free of its pulley and struck the employee's left ankle, resulting in a fractured ankle and injured thumb.
