29 CFR 1918.24(c): Portable straight ladders were not of sufficient length to extend 3 feet (0.91m) above the upper landing surface. Onboard the MV Frauke, Hatch - It was observed on 10/20/11 that a portable ladder did not extend 3" above the landing surface.

29 CFR 1918.32(b): The edge of a hatch section, or of stowed cargo, was not guarded by a vertical safety net or other means providing equal fall protection to exposed employees. Onboard MV Frauke, Hatch - On 10/20/11 two longshoremen were observed working on top of cargo within 36" from the edge without fall protection. The measured height of the fall was approximately 13 feet to the landing surface.
