29 CFR 1910.23(a)(7):     Every temporary floor opening did not have standard railings, or was not constantly attended by someone:    Factory Deck:  On or about February 12, 2012, and at times prior thereto, the freezer hatch was guarded by a single rope "rail" place at a height of 40 inches.  This condition exposed employees to a height of 11 feet.

29 CFR 1910.133(a)(1):     The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    Factory Deck:  On or about February 12, 2012 and at times prior thereto, employees were not provided with chemical splash goggles when using the cleaner "Super Red Hot."  This condition exposed employees to eye hazards from the corrosive liquid.
