OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.  F/V Alaska Mist, Machine shop area, Coastal Dock:  On or about 4 February 2015  and times prior thereto the Blue Miller 180 welder was improperly wired to a 208 volt power cord.  This condition exposed employees to an electrical shock and burn hazard.

(a)  F/V Alaska Mist, Factory Deck Area, Coastal Dock:  On or about 4 February 2015 and times prior thereto, the rope railing was not taunt and had slack.  This condition exposed employees to an eight foot fall hazard.   (a)  F/V Alaska Mist, Coastal dock, Engineering deck area:  On or about 4 February 2014 and times prior thereto, the 30" X 36" hatchway was opened and not guarded.  This condition exposed employees to a 103" fall hazard.
