F/V Alaska Mist, Machine shop area, Coastal Dock: On or about 4 February 2015 and times prior thereto the Blue Miller 180 welder was improperly wired to a 208 volt power cord. This condition exposed employees to an electrical shock and burn hazard.

This condition exposed employees to a 103" fall hazard.
