29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs(b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. (a) F/V Alaska Mist (Machine Shop): On or about 2/12/2012, and times prior thereto, the Dayton Bench grinder did not have a tongue guard; exposing employees to struck by hazards in the event of wheel failure. (b) F/V Alaska Mist (Reefer Flat): On or about 2/12/2012, and times prior thereto, the Dayton Bench grinder did not have a tongue guard; exposing employees to struck by hazards in the event of wheel failure.

Such work rests shall be kept a distance not to exceed 1/8 inch from the surface of the wheel. (a) F/V Alaska Mist (Machine Shop): On or about 2/12/2012, and times prior thereto, the Dayton Bench grinder work rest measured 1/4 inch; exposing employees to struck by hazards in the event of wheel failure. (b) F/V Alaska Mist (Reefer Flat): On or about 2/12/2012, and times prior thereto, the Dayton Bench grinder work rest measured 1/4 inch; exposing employees to struck by hazards in the event of wheel failure.

29 CFR 1915.56(b)(2): Only cable free from repair or splices for a minimum distance of ten (10) feet from the cable end to which the electrode holder is connected shall be used, except that cables with standard insulated connectors or with splices whose insulating quality is equal to that of the cable are permitted. F/V Alaska Mist (Reefer Flat Area): A welding lead was available for use which had been damaged within 10 feet of the electrode holder; exposing employees to electrical hazards.

29 CFR 1918.62(a)(2): Defective gear, as defined by the manufacturers' specifications (when available), shall not be used. Distorted hooks, shackles or similar gear shall be discarded. Weather Deck: On or about February 13, and times prior thereto, brailer bags were used with cuts and snags on the bag and lifting legs. This exposed employees to struck-by hazards from defective gear failing during offloads of up to 2000 ilbs of fish.
