All energy isolating devices that are needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). a) On or about 4/26/12 in the Palletizer area; the three ball valves identified in Lockout-Tagout Procedure ID# L1-B1-Palletizer were not turned to the off position as part of the energy control methods prior to performing servicing and maintenance work, such as but not limited, to removal and clean up of crashed pallets.
