29 CFR 1910.132(d)(1)(i): The employer did not select and have each employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:    a) On or about 01/05/15 in the Line #3 area; employer failed to protect employee(s) from chemical flash and splash hazards to the face during the use of ZEP Cleaner ( R-12776 - corrosive ), when they only selected goggle use when handling this chemical.       ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a) On or about 01/05/15 in the area near Line #3; employer failed to protect employees from chemical burns when handling ZEP brand R-12776 cleaner ( corrosive solid ) to clean the filler tank, when the two closest eye wash/safety shower locations were not adjacent to the hazard, they were located on the Kitchen Mezzanine and between Lines #1 & #2 near the labeler.     ABATEMENT DOCUMENTATION REQUIRED
