29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 07/19/18 On Line #6, Lee Road facility; employees were exposed to an amputation hazard when they crawled under the ABC Decaser and reached into the conveyor to clear multiple jams (7-8 jams within 20 minutes)of cardboard boxes and the employer did not require the use of the written ABC Decaser hazardous energy control procedure. NO ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a) On or about 07/19/18 On Line #6, Lee Road facility; employees were exposed to an amputation hazard while clearing multiple jams of cardboard boxes on the decaser when the employer training did not provide adequate information regarding when to implement the steps indicated in the control of hazardous energy procedure, i.e. preparation for shutdown, shutdown, isolation, application of control, verification of zero energy condition. NO ABATEMENT DOCUMENTATION REQURIED
