29 CFR 1910.147(c)(4)(ii):     The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section:    a. Although a general lockout program was written (but not updated), there were no specific procedures, as required by 1910.147(c)(4)(ii)(A) through (D), including identification of types and magnitude of energy sources, how the machine is to be powered down and the specific location as to where a lock is to be placed, specifically how the machine is to be tested to ensure it is locked out, and any other specific procedures for the machine identified.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

An annual inspection (audit) of the lockout program was not done (the general lockout program did identify the need for an annual inspection).    Abatement certification and abatement documentation is required for this violation. 29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a. The unused portion of a band saw was not guarded.    b. The light curtain, on a Cincinnati press brake, was not always tested according to the manufacturer's requirements before each day's use.
