On or about October 17, 2023, and prior thereto, SHURE MANUFACTUING CORPORATION did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment (PPE). Employees were exposed to hazards including but not limited to flying particles from operating a mitering saw to cut cardboard, which necessitated the use of eye protection.

29 CFR  1910.178(l)(2)(ii): Powered industrial truck training had not consisted of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  On or about October 17, 2023, and prior thereto, SHURE MANUFACTURING CORPORATION did not ensure that each powered industrial truck (PIT) operator received training that consisted of formal instruction, practical training, and evaluation of the operator's performance in the workplace. Employees were exposed to struck-by, caught-between, and crush hazards from untrained PIT operators operating PIT equipment.

29 CFR  1910.178(q)(7): Industrial trucks had not been examined before being placed in service, and shall not be placed in service if the examination showed any condition adversely affecting the safety of the vehicle. Such examination had not been made at least daily.  On or about October 17, 2023, and prior thereto, SHURE MANUFACTURING CORPORATION did not conduct pre-shift inspections of each powered industrial truck before use, at least once daily. Therefore, employees were exposed to struck-by, caught-between, and crush hazards.
