29 CFR 1910.151(c): The employer did not ensure that where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body was provided within the work area for immediate emergency use.      a.) Deptartment 13, Nickel/Chrome plating area - On or about February 22, 2013, and at times prior thereto, the emergency eyewash in the immediate area was not operable.  Company employees in Department 13 worked with and around corrosive chemicals including, but not limited to, sulfuric acid, sodium copper cyanide and sodium hydroxide.

29 CFR 1910.1200(f)(5)(i): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemical(s) contained therein:    a.) Department 13, Nickel/Chrome plating area - On or about February 11, 2013, an employee adding chemicals to the plating tanks utilized 2 white 5-gallon buckets, one contained sulfuric acid and the other contained caustic liquids, that were not labeled to identify the product contained therein.  Sticky notes were utilized on the bucket lids to identify the chemical within.  After the lids were removed, the employee mistakenly added sulfuric acid to the copper cyanide tank causing a chemical reaction that splashed corrosive chemicals out of the tank and onto the employee, resulting in second degree chemical burns.

29 CFR 1910.1200(f)(5)(ii): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the appropriate hazard warnings:   a.) Department 13, Nickel/Chrome plating area - On or about February 11, 2013, an employee adding chemicals to the plating tanks utilized 2 white 5-gallon buckets, one contained sulfuric acid and the other contained caustic liquids, that were not labeled to identify the hazard warnings of the chemicals contained therein.
