29 CFR 29 1910.178(m)(5)(i): Each employee leaving a powered industrial truck unattended was not able to shut the power off, and set the brakes.    a) On or about May 3, 2013, and at times prior thereto, employees were exposed to struck-by hazard while operating a Linde forklift #6 that would not shut off, after the ignition key was turned off and removed.    b) On or about May 3, 2013, and at times prior thereto, employees were exposed to a struck-by hazard while operating a Linde forklift #2 with the parking brake lever tied to the forklift frame, in the unlocked position so it could not be set.
