29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:  Main building, production lines, production machinery including fillers, labelers, lidders, cappers: on October 24, 2013, authorized employees locking out production machinery for changeovers, servicing and other setup tasks were not inspected periodically (at least once per year) to ensure their level of proficiency in the hazardous energy control procedures implemented by the employer to ensure sufficient level of protection and compliance.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:   Main building, production department line #7, Pneumatic Scale 20 Head Filler: on or about October 24, 2013, designated employees cleaning filler's head nozzles and center bells were exposed to amputation and laceration hazards when hazardous energy was not isolated and machine remained energized through the main plug connection.  There is no abatement certification or documentation required for this item.  The Sigan America LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(7)(i)(A), which was contained in OSHA inspection number 314039736, citation number 01, item number 002 and was affirmed as a final order on 03/17/2011, with respect to a workplace located at 1111 W. McKinley Rd., Ottawa, IL 61350.
