29 CFR 1910.303(b)(1): Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the following considerations:  29 CFR 1910.303(b)(1)(ii): Mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided;  a)a) In the Setpack Department, the safety shut-off electrical drawer of the A-1 tape machine had opened exposing employees to live electrical parts.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means:  a) In the Setpack Department, the safety shut-off electrical drawer of the A-1 tape machine had opened exposing employees to live electrical parts.

29 CFR 1910.134(c)(2)(i): An employer may provide respirators at the request of employees or permit employees to use their own respirators, if the employer determines that such respirator use will not in itself create a hazard. If the employer determines that any voluntary respirator use is permissible, the employer shall provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard"); and         a) In the Setpack Department, employees provided respirators to wear when the building's heaters were first turned on at the on-set of cold weather were not provided the information contained in 1910.134 Appendix D prior to respirator use.
