29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.  a)  Corbin Avenue & Jerome Drive job site - On or about 10-21-13 - Employee was observed not wearing eye protection while using a Hitachi pneumatic nail gun to erect wood framing, exposing him to eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  Corbin Avenue & Jerome Drive Job Site - On or about 11-21-13 - Employees were observed working on a second floor deck installing an exterior wall section with no fall protection in use. Employees were exposed to a ten foot fall hazard.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge.  a)  Corbin Avenue and Jerome Drive job site - On or about 11-21-13 -  Employees were using a wooden interior stairway to access the second floor of the building that was seven risers in height with no standard railing installed on the interior side.
