29 CFR 1910.95(i)(3): Employees were not given the opportunity to select their hearing protectors from a variety of suitable hearing protectors provided by the employer:  a.    An employee, operating the tucker machine, was exposed to a noise dose of 72.8 %, or an equivalent dBA (decibels A scale) of 87.71 dBA, which is capable of causing hearing loss. The sampling was performed for 445 minutes during one shift on January 15, 2014.   The employer only provided one type of hearing protection, SafeDirector foam ear plugs.

On or about January 14, 2014, the company had a 275-gallon of container of Blue DEF, a mild irritant, located outside of the maintenance shop that was not labeled, tagged or marked to show the identity of the hazardous chemical contained therein.
