29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  a) On or about August 10, 2019, and at times prior, the employer did not require workers to submit to a medical evaluation prior to their participating in a voluntary-use respiratory protection program. 29 CFR 1910.305(g)(1)(iv): Flexible cords and / or cables were used as a substitute for the fixed wiring of a structure:  a) North end of 3090: On or about August 10, 2019, and at times prior, the employer [A) used a flexible cord as permanent wiring where fixed outlets were not available, and [B] passed the cord through a wall opening.    Abatement Note: The employer provided proof of abatement.
