29 CFR 1910.180(h)(2)(ii): The load was not attached to the hook by means of slings or other approved devices.   Drydock:  Employees used a personnel lifting platform with screw pin shackles connected on four attach points of the platform, that were not positively secured. This exposed employees to a potential serious fall hazard from the lifting platform if the pin on the shackle backed out.

29 CFR 1915.112(c)(2): Thorough chain inspections were not conducted every three months.  Maintenance shop:  Lifting sling chains were not being thourghly inspected every three months.  This exposed employees to struck-by hazards if a chain broke due to defect on the chain.
