29 CFR  1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :   a) At SOMAR shipyard throughout the facility, located at 40 Ross Inlet Road, Coos Bay OR: On 04-26-2023, the employer failed to conduct an annual audiogram due in January 2022, for each employee exposed to high levels of occupational noise. Employees were exposed to continuous and intermittent occupational noise levels of 92 dbA to 115 dbA. Failure to conduct an annual audiogram exposed employees to a potential aggravation of their hearing loss if prophylactic hearing conservation measures are not implemented to protect an employee suffering a Standard Threshold Shift.
