29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazards:     a) Oxide Area - An employee used a retractable lanyard for fall protection while working on top of railroad cars, but the anchorage beam provided was not long enough. The employee would pull a long length of lanyard out when working on the end hatch, which would allow the employee to hit the ground if he fell off the car, reported on or about 2-20-14.    Abatement Certification Required.

29 CFR 1910.178(q)(6): Powered industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, by the addition of extra parts not provided by the manufacturer, or by the elimination of any parts without the manufacturer's approval:     a) Main Department - An employee operated a Caterpillar V60E forklift and a Caterpillar 5000 forklift, as needed, while the trucks had replacement horns installed in the left panel on #10 and on the column of the # 45 truck.  Both forklift trucks did not have their horns in these locations when they were manufactured, observed on or about 2-20-14.     Abatement Certification Required
