29 CFR 1910.23(a)(2): Every ladderway floor opening or platform shall be guarded by a standard railing with standard toeboard on all exposed sides (except at entrance to opening), with the passage through the railing either provided with a swinging gate or so offset that a person cannot walk directly into the opening.   a. At the workplace in the upstairs office area, a stairway landing did not have a standard toeboard to protect employees traveling underneath from falling objects, exposing them to struck-by hazards.

29 CFR 1910.24(h): "Railings and handrails." Standard railings shall be provided on the open sides of all exposed stairways and stair platforms. Handrails shall be provided on at least one side of closed stairways preferably on the right side descending. Stair railings and handrails shall be installed in accordance with the provisions of 1910.23.   a. At the workplace in the upstairs office area, a standard railing was not provided on all open sides of a staircase that led to upper level offices, exposing employees to fall hazards.

You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  a. At the facility located at 436 12th Street NE, Strasburg, Ohio, at the time of inspection, the employer had not recorded information in column (F) correctly for calendar year 2014 a description of work-related injuries and illnesses. The employer did not identify the object/substance that directly injured, or made each person ill.
