Erie Plating Company, 656 West 12th Street, Erie, PA 16501, barrel line area, on or about March 20, 2014 and times prior thereto: Employees access a mezzanine area to reset breakers on rectifiers. The mezzanine platform was not equipped with a standard guardrail system when performing these tasks, and employees were exposed to a 12.1-feet fall hazard. B.) Erie Plating Company, 656 West 12th Street, Erie, PA 16501, anodizing area, on or about March 20, 2014 and times prior thereto: Employees access a mezzanine area to reset breakers on rectifiers. The mezzanine platform was not equipped with a standard guardrail system when performing these tasks, and employees were exposed to a 10-feet fall hazard. IN THE ALTERNATIVE: 29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: A.) Erie Plating Company, 656 West 12th Street, Erie, PA 16501, barrel line area, on or about March 20, 2014 and times prior thereto: Employees access a mezzanine area to reset breakers on rectifiers. The damage included, but was not limited to, fraying to the outer jacket of the flexible cord where the cord ended into the vibrator. B.) Erie Plating Company, 656 West 12th Street, Erie, PA 16501, maintenance shop area, on or about April 1, 2014, and times prior thereto: The flexible cord for a Modine heater fan was damaged. The damage included, but was not limited to, fraying to the outer jacket of the flexible cord at the cord cap. C.) Erie Plating Company, 656 West 12th Street, Erie, PA 16501, maintenance shop area, on or about March 20, 2014 and times prior thereto: The flexible cord for a light on the Work-A-Matic band saw was damaged. The damage included, but was not limited to, a slit in the outer jacket of the flexible cord near the cord cap.
