29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling. Fabrication area; on or about 1 April 2014 and times prior thereto; an electrical wire measuring approximately 48 inches in length, leading from an electrical junction box had a receptacle plug attached to an extension cord. Wiring type was boat cable 600V 105 degrees Celsius dry, 75 degrees wet, oil resistant, BC5W2 E67078, LL22035, CSA, AWM, II A/B. Wiring was externally fastened to existing conduit with zip ties. This condition exposed employees to electrical shock hazard.

Shop bay, welding area adjacent to safety office; on or about 1 April 2014 and times prior thereto; the employer did not ensure that a Miller Arc Welder, Model SRH-444, Stock Number 900750, Serial Number JK535478, electrode holder cable was free of repairs. The electrode cable outer jacket was damaged and covered with electrical tape 6 feet and 6 inches from the end of the cable. This condition exposed employees to an electrical shock hazard.

Fabrication area; on or about 1 April 2014 and times prior thereto; the work rest on a Dewalt 8 inch heavy duty bench grinder, serial number unreadable, was not properly adjusted to a maximum of 1/8 of an inch from the abrasive wheel. The gap between the abrasive wheel and work rest measured 3/8 of an inch. This condition exposed employees to a caught in hazard.

Four instances were noted: a. Sysco Creme Cleanser, 40 ounces. b. Nautical Ease Inflatable Boat Cleaner, 32 ounce. c. Rejuvenate Cabinet and Furniture Restorer and Protectant, 16 ounces. d. Meguiars Deep Crystal Car Wash, 64 ounces. This condition exposed employees to irritation of the eyes and upper respiratory tract, and skin dermatitis hazards.
