Shop bay adjacent to safety office; on or about 07 May 2019 and time prior thereto; the employer did not ensure that guards were provided on machinery. A Jet Gear Head Bench Lathe, model GHB-1340, Serial number 6060155B did not have the chuck guard installed. This condition exposed employees to a struck by hazard.

29 CFR 1915.134(b): Work rests were not kept a distance not to exceed 1/8 inch from the surface of the wheel.    Shop bay adjacent to safety office; on or about 07 May 2019 and times prior thereto; the work rest on a Craftsman bench grinder, model 397-19450, serial number unknown, was not properly adjusted to a maximum of 1/8 of an inch from the abrasive wheel. The gap between the abrasive wheel and work rest measured 3/8 of an inch. This condition exposed employees to a caught in hazard.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:  Shop bay compressed gas storage area; on or about 07 May 2019 and times prior thereto; the employer did not ensure that compressed gases cylinders were secured. Fifteen compressed gas cylinders of various quantities were stored free standing. This condition exposed employees to a struck by hazard.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and/or cables were run through holes in walls, ceilings, or floors.  South end of shop bay, on or about 07 May 2019 and times prior thereto; four separate energized electrical cords were route through a wall opening. One red and two black heavy duty 50-foot energized extension cords and a blue medium duty energized 50-foot extension cord was routed through the metal hole opening in the sliding door. This condition exposed employees to an electrical shock and burn hazard.
