29 CFR 1910.95(i)(5): The employer did not ensure proper initial fitting and supervise the correct use of all hearing protectors:   (a) On or about April 9, 2014, in the final assembly area, for an employee using a pneumatic hand held buffer to buff fiberglass reinforced plastic bus panels and parts, that was exposed to a dose of 135% measured on a 80 dBA threshold dosimeter. This was equivalent to a sound level of 92 dBA. Sampling was conducted on April 9, 2014 for 311 minutes. Zero exposure was for 169 minutes to total 480 minutes. The worker was wearing Moldex disposable plugs that were not correctly inserted into his ear canals.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:     (a) On or about April 8, 2014, in the final assembly area on the sanding/buffing table, employees were pouring Luperox DDM-9, a ketone peroxide catalyst from a quart size bottle into a plastic cup for mixing with resin. The nearest eye wash station was more than 188 feet away from this table in the adjacent wet manufacturing area.

29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d):    (a) On or about April 9, 2014, in the trim booth, for an employee trimming fiberglass reinforced plastic parts with hand held pneumatic grinders, exposed to a time weighted average of 37.09 mg/m3 of total dust. Sampling was conducted for 370 minutes. Zero exposure was assumed for 110 minutes to total 480 minutes.   Feasible engineering and administrative controls available include but not limited to the installation of a dust collection system, using tools equipped with local exhaust, eliminating the use of compressed air to clean off dust from clothes and other surfaces, and the use of employee rotation.
