On or about June 4, 2014, on the west side of a new construction condominium complex building, the employer did not ensure holes in walking working surfaces were covered; employees were not protected from fall hazards up to 10 feet 9 inches. b. On or about June 4, 2014, on the east side of a new construction condominium complex building, the employer did not ensure holes in walking working surfaces were covered; employees were not protected from a fall hazard of 14 feet.

On or about June 4, 2014, employees framing a new construction condominium complex were using a job made ladder that was not one-quarter of the working length of the ladder, exposing employees to a 9 feet 8 inch fall hazard.
