29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.     a.   On or about June 4, 2014, on the west side of a new construction condominium complex building, the employer did not ensure holes in walking working surfaces were covered; employees were not protected from fall hazards up to 10 feet 9 inches.      b.  On or about June 4, 2014, on the east side of a new construction condominium complex building, the employer did not ensure holes in walking working surfaces were covered; employees were not protected from a fall hazard of 14 feet.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder:    a.   On or about June 4, 2014, employees framing a new construction condominium complex were using a job made ladder that was not one-quarter of the working length of the ladder, exposing employees to a 9 feet 8 inch fall hazard.
